Some clients arrive expecting the first session to end with a label — an answer to “what’s wrong with me.” We usually don’t lead with that, and it’s a deliberate choice, not an oversight. A diagnosis is a billing and treatment-planning tool as much as a clinical one; it’s useful, but it’s not the most useful thing we can do with fifty minutes on day one.
What we do instead: listen for the actual texture of what’s going on, what’s already been tried, and what a good outcome would look like to you specifically. The diagnostic picture usually becomes clear over the first few sessions anyway, once there’s enough information to make it accurate rather than approximate.
